Checklist – Lasting Powers of Attorney
Before you begin, please ensure that you have the following information to hand:Details of your Attorneys and any Reserve Attorneys whom you wish to appoint (with their permission)Details of your Certificate Provider – this is a separate person with whom you have discussed your Lasting Power of Attorney, and whom you have known for at least two years. Your Certificate Provider can not be one of your Attorneys.Details of any specific wishes or preferences in relation to your healthcare treatment or financial circumstances (optional).Details of the person/people who will witness your signatures later.Details of any person that you wish to be notified when your LPA is registered (optional).A scan or photograph or your passport or driving licence to send to us as ID. Please contact us if you do not have a passport or driving licence.
